Cancer Diary: Is a Neck Lump or Swelling a Sign of Cancer?

 


A lump in the neck is one of those discoveries that stops time. You touch your throat, feel something that wasn’t there before, and your mind leaps to the worst possibility: Is it cancer?

That question deserves both honesty and calm.

1. The neck is a crossroads

The neck is a busy intersection of glands, lymph nodes, muscles, and blood vessels. It’s where the body filters infection, drains fluid, and manages hormones. Because of that, lumps and swellings are common — and most are not cancer.

Common benign causes include:

  • Swollen lymph nodes from infection (cold, flu, dental abscess, tonsillitis)

  • Thyroid nodules (often harmless)

  • Cysts (fluid-filled sacs)

  • Salivary gland inflammation

  • Muscle or scar tissue thickening

But the same crossroads is also where head and neck cancers can appear — quietly, at first.

2. When a lump deserves closer attention

Doctors often look for patterns rather than panic. A lump is more concerning when it:

  • Persists longer than two weeks

  • Grows instead of shrinking

  • Feels hard, fixed, or irregular

  • Is accompanied by pain, difficulty swallowing, voice changes, or ear pain

  • Appears with unexplained weight loss or fatigue

These are not diagnoses — they are signals. The body’s way of saying, “Please look closer.”

3. The lymph node story

Lymph nodes are the body’s sentinels. They swell when fighting infection — that’s their job. But sometimes, they swell because they are infiltrated by cancer cells.

Cancers that can cause neck lumps include:

  • Lymphoma (cancer of the lymphatic system)

  • Metastatic head and neck cancers (from mouth, throat, or thyroid)

  • Leukemia

  • Thyroid cancer

  • Salivary gland cancer

The key difference is persistence. Infection-related nodes usually shrink within weeks. Cancer-related nodes do not.

4. The thyroid connection

The thyroid sits low in the neck, and nodules are common — especially in women. Most are benign, but some can be malignant. Warning signs include:

  • Rapid growth

  • Hoarseness

  • Difficulty swallowing

  • Family history of thyroid cancer or radiation exposure

A simple ultrasound and blood test can clarify what’s happening.

5. The emotional layer

Finding a lump triggers fear before facts. That fear is valid. But fear should lead to action, not paralysis.

The right next step is evaluation, not assumption. A doctor can examine, image, and — if needed — biopsy. The process is straightforward, and early detection changes everything.

6. The takeaway

Not every lump is cancer. But every lump deserves respect.

The body speaks in textures and shapes. A lump is one of its clearest words. Listen, but don’t leap. Investigate, but don’t catastrophize.

Cancer hides best in silence. The moment you ask the question — “Is this normal?” — you’ve already broken that silence.

A Publisher's Conversation with Authors: De-confusing Royalty Structure

  Authors frequently confuse LIST and NET and, in this way, end up with contracts that pay lower royalties when they believe that they are getting a good deal. Often, they cannot negotiate in ways that benefit themselves because they do not understand this difference and its significance. Let's break this down. List refers to the retail price of the book. Payment on list means a percentage of the retail price of the book. If royalties are 8% of list (5-8% is typical), then on a $10 book, royalties would be 80 cents; if they are 10%, royalties are $1, on every book sold. Easy to calculate, easy to predict income, and always standardized, no surprises or overestimations. Net refers to what is leftover after expenses.
